The Fraser Health Center is the Skilled Nursing and Rehabilitation component of the Seabrook. Our Seabrook residents enjoy priority access to health care services.

The Fraser Health Center is a licensed, Medicare-certified, skilled nursing facility with physical, occupational and speech therapy services. Although we are a small 33-bed facility, we provide all residents with private accommodations. We strive to promote a home-like environment for our residents.

Twenty-four hours a day our professional staff is dedicated to our philosophy of assisting residents, both short and long term, to achieve and maintain their optimal level of functioning. We respect the dignity of each individual, and recognize their physical, mental and emotional needs and rights.

In October 2012, The Fraser Health Center received a 5-Star Rating (only facility in Beaufort County and only one in a five county radius) by the Centers for Medicare and Medicaid (CMS) and is consistently one of the top-ranked Medicare Skilled Nursing Facilities in the US (www.NursingHomeCompare.gov.)  In February 2013, US News reported there are 43 nursing homes in the Lowcountry, seven of which received an overall rating of Five Stars from the Federal Centers for Medicare & Medicaid Services.  Fraser is one of those seven.  http://health.usnews.com/best-nursing-homes
